National’s “Passport Rehabilitation” program was specifically designed to meet the needs of individuals requiring a short-term rehabilitative stay following a hospitalization or an acute medical episode. While on your short-term “trip” with us, please take full advantage of the amenities and services our facilities offer. During your stay, if you find anything requiring our attention, please let us as soon as possible so that we may make your stay the best it can be.

Some of the amenities offered in the Passport Program found at our facilities, in addition to a Clinical Nurse Specialist to oversee your entire stay, the following amenities are offered to make your stay more comfortable:

  • Electric beds

  • Basic Cable TV with remote

  • DVD player / library

  • Bedside Clock Radio/CD Player

  • Hair Dryer

  • Wireless Internet (Wi-Fi) access

  • Telephone

  • Daily newspaper delivery

  • Magazines

  • Selective Dining and/or Room Service

 


Maple View Manor has the capability of providing care to individuals with a wide range of wound care needs. The wound care management program is designed to promote maximum wound healing by utilizing a interdisciplinary team approach to treatment and patient education. Initial and concurrent treatment modalities and adjunctive therapies are provided to insure the best possible outcomes and healing rates.

This program is designed to provide medical management and rehabilitation to individuals with a new onset of neurological condition (s) affecting multiple levels of functioning. An interdisciplinary team approach provides comprehensive treatment to facilitate maximum independence and compensatory training for both the patient and caregivers. Individuals being referred for this program should be medically stable and exhibit potential for functional gains. Patients should be able to actively participate in skilled therapies 5 -7 days a week.

Maple View Manor has the capability of providing care to many types of individuals needing post-surgical interventions. The program is designed for individuals with complex care issues requiring aggressive skilled nursing care and an interdisciplinary approach to care. Individuals should generally be stable and have the ability to actively participate with the integration of medical services and rehabilitation.

This program is designed to provide comprehensive monitoring and management of patients following acute hospitalizations for cardiovascular conditions. This would include those patients that have been treated for medical conditions and/or have undergone surgical procedures.

To ensure a standardized, integrated team process for management of patients with a cardiac diagnosis, the program is designed to provide a graded, progressive and predictable format for treatment.

Maple View Manor has the capability of providing care to individuals with a range of physiological conditions requiring orthopedic intervention and or management. Individual candidates must have the ability to actively participate in rehabilitation and tolerate one to three hours of skilled therapy seven days a week. Sufficient support systems and or a predetermined discharge placement is essential to facilitate a successful and functional outcome. This subacute program is designed for those individuals who require more intensive therapy than is practical at home or in an ambulatory setting.

Medical Services
Our mission is to provide comprehensive medical management to individuals with dementia and Alzheimer's related diseases and to promote opportunities which are individualized to each resident's physical, social, emotional and spiritual needs.

This program represents a unique departure from traditional geriatric long term care. Utilizing specialty selected and educated staff ensures for the provision of a calm, predictable, comforting and safe environment.

60 bed Alzheimer's/Dementia Unit, specializing in the provision of care for both higher functioning, as well as, compromised individuals under the guidance of a program director.

Utilizes an Interdisciplinary Professional Team

  • A Board Certified Psychiatrist, as part of the interdisciplinary team, provides direction as needed for medication and behavioral management

  • Incorporates family in resident care planning
  • Treatment team meeting coordinated by program director in conjunction with resident care plan coordinator

  • Promotes optimum physical functioning utilizing our rehab team
  • Provides family, staff and general public opportunities to learn about Alzheimer's Disease
  • Provides respite and permanent placement for Alzheimer's/Dementia population

  • Obtain physician recommendation for placement

  • Resident admission accepted
